Conviction Date Submission Form

Please use this form to request submission of your 508 form to the NC DMV. Include the office of A Fresh Start where you did your treatment or ADETS.

If your treatment was outside of NC you can contact us for an Out of State Review so that we can file your 508 for you.
If another NC DWI facility did your treatment or ADETS then contact that facility for your 508 form submission.

We will send your 508 after we verify your completion and payment. Processing should take 2-5 days once we submit the 508 to the DWI department of NCDHHS.
